چکیده

P a t i e n t s 16 588 patients who were ≥ 18 years of age (mean age 61 y, 75% men) and had continuous symptoms of chest discomfort for ≥ 30 minutes and < 6 hours from onset to the time of randomization, and electrocardiographic criteria of ST-elevation MI or new left bundle-branch block. Exclusion criteria were planned catheter-based reperfusion; active bleeding or a noncompressible vascular puncture site; systolic and diastolic blood pressures > 180 mm Hg and 110 mm Hg, respectively; warfarin therapy; stroke in the previous 2 years; weight > 120 kg; or platelet count < 100 000 cells/μL. Follow-up was 98%.

Reperfusion therapy for acute myocardial infarction with fibrinolytic therapy or combination reduced fibrinolytic therapy and platelet glycoprotein IIb/IIIa inhibition: the GUSTO V randomised trial.

BACKGROUND Plasminogen activator therapy for acute myocardial infarction is limited by lack of achievement of early, complete, and sustained reperfusion in a substantial proportion of patients. Many phase II trials have supported the potential of combined fibrinolytic therapy and platelet glycoprotein IIb/IIIa inhibition for improving reperfusion. We did a randomised, open-label trial to compar...

Early administration of reteplase plus abciximab vs abciximab alone in patients with acute myocardial infarction referred for percutaneous coronary intervention: a randomized controlled trial.

CONTEXT The optimal pharmacological strategy for bridging the delay between admission and performance of percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (MI) is not known. OBJECTIVE To assess whether early administration of reteplase plus abciximab produces better results compared with abciximab alone in patients with acute MI referred for PCI. DESIGN, ...

Prognostic implication of activated partial thromboplastin time after reteplase or half-dose reteplase plus abciximab: results from the GUSTO-V trial.

AIMS To evaluate the relationship between activated partial thromboplastin time (aPTT) and clinical outcomes in the Global Use of Strategies to Open Occluded Coronary Arteries (GUSTO-V) trial comparing standard-dose reteplase to half-dose reteplase and abciximab. METHODS AND RESULTS We analysed data on 11,420 patients receiving unfractionated heparin. Peak aPTT levels recorded during the hosp...

Combination reperfusion therapy with abciximab and reduced dose reteplase: results from TIMI 14. The Thrombolysis in Myocardial Infarction (TIMI) 14 Investigators.

Aims Abciximab has previously been shown to enhance thrombolysis and improve myocardial perfusion when combined with reduced doses of alteplase. The purpose of the reteplase phase of TIMI 14 was to evaluate the effects of abciximab when used in combination with a reduced dose of reteplase for ST-elevation myocardial infarction. Methods and Results Patients (n=299) with ST-elevation myocardial i...

Facilitation of early percutaneous coronary intervention after reteplase with or without abciximab in acute myocardial infarction: results from the SPEED (GUSTO-4 Pilot) Trial.

OBJECTIVES We examined the utility of early percutaneous coronary intervention (PCI) in a trial that encouraged its use after thrombolysis and glycoprotein IIb/IIIa inhibition for acute myocardial infarction (MI). BACKGROUND Early PCI has shown no benefit when performed early after thrombolysis alone. METHODS We studied 323 patients (61%) who underwent PCI with planned initial angiography, ...
